Complete the DS-11

To clarify, the Complete the DS-11 form filler tool isn’t an online passport application. Instead, it allows you to input your data electronically and generate a paper form with a unique barcode. After completing the form electronically and printing it, you’ll need to submit it either by mail or in person, depending on the form type (DS-82 and DS-5504 forms by mail; DS-11 form in person).

Since the data you input is directly linked to your application’s unique barcode. Please avoid making any handwritten changes, corrections, or additions to the data or the form after printing it. Mainly, as this could lead to errors and processing delays. If you spot an error or leave a field blank after printing, it’s best to start over and complete a new form.

An important note for families: refrain from using software programs or tools to transfer or copy data from one family member’s passport application form to another. This as each barcode and set of information is tied to a single applicant’s data.

For the DS-11 form, hold off on signing your name. You’ll need to take the oath and sign the form in front of a passport acceptance agent. This at a designated facility or in front of a U.S. Department of State employee at a passport agency or center. So now you now to complete the DS-11.

For DS-82 and DS-5504 forms, be sure to sign and date the application with the date you’re completing the online form. Applications lacking your signature and date won’t be processed and will be returned to you.

Once you print your form, your information isn’t saved by the form filler and can’t be accessed again. Therefore, it’s crucial to double-check your answers as you won’t be able to correct errors after printing the form.
